Compute DC within chroma-from-luma prediction

David Michael Barr requested to merge barrbrain/dav1d:dc-in-cfl-pred-1 into master

Drop the dual-plane version of CfL prediction and merge in DC prediction for the single-plane version. This fixes #36 (closed) for which merging DC prediction was outstanding.
