Clarify --width and --height

This reflects the actual behaviour of the options.

Alternatively, we could define those options to affect the video size when window fitting is disabled, but that is not the current behaviour. It would also be harder to explain/describe. Remember that fitting is enabled by default, and rarely disabled.
