kva: fix RGB mapping

KO Myung-Hun requested to merge komh/vlc:kva-rgb into master

Fixed:

  1. On 24-bit and 32-bit RGB, R and B are swapped.
  2. On 15-bit RGB, KVAF_BGR16 is used.

Finally, OS/2 is little-endian system.
